Referral Instructions

Patients wishing to see a physician at PNG must be referred by a Family Physician, Nurse Practitioner, or Specialist Physician. To refer a patient, please fax the following items to one of our office locations. Please note that we require the following information in order to accurately triage referrals:

Renal Abdominal Ultrasound Report (If Already Done)

List of Medications
List of Co-Morbidities
Previous Creatinine Measurements in the Past Two Years (If Available) and Any Available Urine Test Results
